New Patients
If you are new to our practice, there are a few steps that you will need to complete prior to our scheduling your initial intake appointment.
You will receive an initial email notifying you of our ability to accept you as a patient.
In fact, you likely already received it if you are reading this now!
Our secure electronic medical records system, SimplePractice, will also send you an email, asking you to sign in to setup your profile, enter some basic demographics information, and review and sign our consent forms. You will need access to your official identification, insurance card, etc.
You will also need to complete our intake questionnaire within SimplePractice. This information is very important, as it helps us to guide our initial appointment.
This questionnaire will also ask for some information that you may not have readily on-hand, such as family history, etc. Please take the time to find and enter as much information here as you can.
We utilize Headway to assist with billing and insurance for non-BCBS (meaning, non-Anthem, CareFirst, etc.) plans. In order to not overload you with emails, we will first verify that the steps above have been completed and then ask Headway to reach out to you to confirm your information.
We are in-network with the BCBS plans, so no worries there!
Lastly, if you have not already requested a specific day and time for the appointment, we will confirm that information via SimplePractice after confirming receipt of the above. You can also reach out to Dr. Moore via message within SimplePractice with any questions.
